Simply complete an application form and submit it along with the required documents through one of the following channels:

Just complete the supplementary card section in the application form when applying for your new credit card. In case you did not do this, you can request for supplementary cards through one of the following channels:

 At any Samba Branch
 Call 24-hour Sambaphone toll-free on 800 124 2000

 For Saudi Nationals: A clear, enlarged copy of National ID card
 For expatriates: A clear copy of Iqama and passport with complete address in    home country
 For salaried employees: A letter from employer stating date of employment,    current position and monthly income
 For self-employed individuals: Bank statement for the last 3 months prior to date    of application
 For non-Samba account holders: A copy of any of the following; photocopy of a    Credit Card from another bank or letter of employment
 For supplementary card applicants: Copy of National ID or passport as applicable    for Saudi Nationals. Copy of Iqama or passport as applicable for expatriates
 Additional documents may be requested by the bank in some instances
